Description

Our mission is to increase access to healthcare for people with special health needs, people in poorer regions, young people, the elderly and marginalized minorities outside health centers, especially in remote areas of Albania and Moldova. At the same time, our goal is to prevent the health of the population of partner countries with a consequent impact on the work performance of citizens and the country's economy. We try to solve mainly three current development problems: Lack and unavailability of professional medical staff. Insufficient patient care between the first and second doctor's visit. Lack of telemedicine solutions suitable for patients with special health needs. In particular, our Scase solution offers: - Portable, multifunctional and medical device - Digital database of records of patients' vital signs during home hospitalization. - Remote patient management for clinics / hospitals - Analyzes and statistics on the prevalence of diseases and the health status of the population of selected countries. The specific goal is a sustainable concept of remote telemonitoring of patients from a business and health point of view through piloting a Scase solution in cooperation with selected local partners. At the same time, it is in our interest to develop Scase from TRL 6 phase to TRL 8 and also to develop the mobile Scase application. Target group: A: NGOs, public institutions, private clinics; B: patients with chronic diseases (Congestive heart failure with / without Diabetes Mellitus type ½); C: patients in acute and post-acute COVID-19 phase, Post-SARS-COV-2 Syndrome; D: cardiological patients in post-operative care E: Palliative care and elderly care Place of implementation: Southeast Europe Region - Albania and Moldova. The applicant falls into category C, as the project is not primarily aimed at combating climate change or protecting the environment. Despite this, the applicant seeks to contribute to this area, at least to the extent set out below. In terms of addressing gender equality, the applicant belongs to group 2.
